New Patient Guide
How to prepare for your first appointment
To help make your first visit with us as easy as possible, we suggest that you bring:
- Insurance cards and any medical records we may have requested, or ones that your primary physician may want you to bring.
- A list of current medications, including doses, as well as over-the-counter medications and herbal products.
- A family member or close friend to listen to what the doctor has to say.
- A copy of your living will or durable power of attorney for health care, if you have one.
If you haven't already filled out paperwork, you should plan to arrive at the center about 30 minutes before your first appointment so you have time to do so.
When you arrive, please stop at the registration desk on the first floor to check in. You will then be directed to your physician's office. Please be aware that a first visit to the physician may take from one to two hours.
You don't have to make any changes to your diet before your visit unless otherwise instructed.
